Private Case Management services follow a collaborative process of assessment, planning, benefit coordination, and advocacy for options and programs to help meet an individual and the family’s goal for comprehensive care needs; all while promoting quality, and cost-effective outcomes.

Services are designed to help individuals and their families continue to navigate and coordinate the financial, government, and legal programs associated with a disability; even after the parents or primary caregivers have passed on, or is no longer able to assist their loved one.
